REQUIREMENTS FOR RECONSTRUCTION OF REPORT OF BIRTH 

(To be submitted in two (2) sets of Photocopies (XEROX)) 

1.)    Certificate of NO RECORD ON FILE from the National Statistics Office (NSO)

            (Negative Certification)           

2.)    Report of Birth form (FA Form No. 40) from the Philippine Embassy or Consulate

3.)    Birth Certificate with English Translation

4.)    First Passport of the child or Travel document

5.)    Authenticated Marriage Contract of parent by the NSO

6.)    Passport of parents used at the time of birth of the child

a.)    Photocopies of data pages

      - In case of unavailability, execute an Affidavit of Loss/Explanation plus copy of present passport
