REQUIREMENTS FOR RECONSTRUCTION OF REPORT OF MARRIAGE 

(To be submitted in two (2) sets of Photocopies (XEROX)) 

1.)    Certificate of NO RECORD ON FILE from the National Statistics Office (NSO)

        (Negative Certification)

2.)    Report of Marriage Form (FSC 75-94) from the Philippine Embassy or Consulate

3.)    Marriage Contract/Certificate with English Translation

4.)    Marriage License/ Certificate of Legal Capacity to Contract Marriage

5.)    Passport of both parties used at the time of marriage

a.)    Photocopies of data pages

      - In case of unavailability, execute an Affidavit of Loss/Explanation plus copies of present passport
